  • The whole story takes place in a hospital. This hospital has limited medical supplies, occasional power failures, and flooding. This is because the Tōhoku earthquake and tsunami followed by the Fukushima nuclear meltdown happened before the start of the game.
  • Yami is a mysterious Emo Teen who visits the Boy after hours, and talks about how alone he is. It's revealed that Yami is a manifestation of the Boy's fears and resentments, especially after he gets sick and after his father dies.
    Yami: They all leave eventually.
  • Gemma's in a coma, while her husband waits by her side. Mercifully, she wakes up.
  • Tony's son, Benny, dies in an accident while being watched by his older sister, Christina. After this, Tony shuts down as he's unable to find a way to express his feelings. Christina blames herself and believes that her father hates her, and seems to think that for a very long time. When Tony realizes how his daughter feels, he's heartbroken.
  • The music is absolutely beautiful, and the songs comprising Mori no Kokoro hits incredibly hard. But hands down the most painful one to listen to is Mom Broken, where the mother literally breaks down crying before successfully singing Walking Through the Night.
    • The lyrics to Walking Through the Night are heartbreaking on their own: it's basically Mom acknowledging and accepting that her son is going to die.

  • Every Character Death:
    • Just as you complete the tea quest, Kisaburo goes missing. Kazuko's saying "One time, one meeting" means how one moment could be both the first and the last so make the best of it. Talking to Jacky and Panky about Kisaburo reveals that he spent his last moments with Kazuko in the backways, where he has planted flowers for her using what he had taken from the hospital. You later see his spirit in the boat to Rakuen along with the Boy, his father, and the rest who've died throughout the story.
    • Sue dies at the end of your quest. At first you think you're saving her and Puchi, the dog she rescued. Then you see her and the boy traveling through her various marble worlds.
    Now it's time for me to leave
    I'm glad you built this little world with me
    • The above event causes the boy to go into Heroic BSoD. Yami comes to see him, and talks about how the people he loved chose to left him. he begs the Boy to come with him. The Boy, who is grieving, agrees. Then Yami turns into an envoy and vanishes with the boy.
    • The Boy's wish: he asks to go to Rakuen. It's been established that Rakuen is a mystical form of the afterlife.
    • Before he goes on the boat to Rakuen, the boy gives his paper hat to his mother, so that she can have armor for protection. This reveals he's lost all his hair to chemotherapy.
    The Boy: You're my hero.
    • At the very end of the game, the mom goes home to see the boy's younger brother and grandmother. The younger boy finds out that the boy died, and hugs his mother. The final scene is the younger brother and mother taking two boats to a pond and sending them off.
