J.D, frustrated with how slow things are moving with Kylie, ditches their plans when Molly comes back into town. The Janitor enlists the help of Elliot to win a bet against Dr. Cox.
- An Aesop: Think about how what you do will affect the feelings of someone you care about. J.D. blurts out to his girlfriend how he blew her off to hang out with a girl he had a crush on, so she dumps him. Turk neglects to tell an ex-girlfriend he has been speaking with that he is married, worsening his already fractured relationship with Carla. The Janitor lies to Elliot about winning his bet with Dr. Cox to trick her into a date, almost ruining his friendship with Elliot. He manages to narrowly save it by explaining the real reason he has a crush on her but is also forced to watch Dr. Cox completely destroy his van.
- The Bet: Cox and Janitor make a bet over whether Janitor can win blonde doctor's affections.
- Both Sides Have a Point: JD almost cheated on Kylie, and he's been pushing on her boundaries, but he's made it clear that sex is important to him and she's been teasing him with it for a while now (she invited him to bed, then disinvited him when Turk and Carla interrupted).
- Cloud Cuckoo Lander: Janitor and all of his first brain trust. He doesn't know Elliot's name, despite his crush, and as for their ideas, well: "Arson and tugboats, that's all I get from you guys!".
- Downer Ending: JD and Kylie break up and Carla learns that Turk has spent the episode talking to his ex-girlfriend without informing her that he's married.
- Every Car Is a Pinto: Cox puts a brick on the accelerator and sends Janitor's van crashing into a wall.Ted: We can fix that.
Van: [explodes] - Hopeless Suitor: Janitor has a crush on Elliot. He and Cox make a bet on whether he'll get the girl, his van for Cox's Porsche. Cox gets to destroy his van in the end.
- Humble Pie: Elliot is upset when she learns Janitor lied to her about the bet and tricked her into a date. He fesses up and admits that his crush on her is due to the fact that she's the only member of the medical team that treats him like a person.
- Mood Killer: JD can't help but get in his own way. He spills the beans to Kylie that he was out with Molly then admits that he had a crush on her before he met Kylie. They break up.
- Noodle Incident: "I saved you from that eagle, Randall. I saved your job, Troy. And, Margo, I found your birth mother. She was a tree person, remember? Now, there's no shame in that."
- Status Quo Is God: Janitor trades in his original Brain Trust (Randall, Troy, and Crazy Eyes Margo) for a new one (Dough, Ted, and the Todd). He ditches the new team for the old team after he loses the bet to Cox.
