Perry is jealous from the baby taking the attention of Jordan, Elliot tries to sabotage her relation with Paul, Turk and Carla are arguing more after they got engaged, in the middle of all this mess JD feels lonely and starts a weird relation with TCW which stands for Tasty Coma Wife, a wife of a patient in coma that starts to feel alone so goes out with JD but then they decide that she needs to take more time.
- Alone in a Crowd: JD ends the episode alone in the hospital, surrounded by the sped up zooms of his colleagues.
- Audience Surrogate: Laverne walks in after JD's rant, asking if she missed something good. She consistently enjoys the antics of the show as "better than her stories".
- Beware the Nice Ones: JD, usually the nicest and most easygoing of the entire staff, ends up snapping at everyone when they call him out for being interested in a coma patient's wife. He ends up calling out how they have no place to criticize who he goes out with given how poorly their own relationships are, Dr. Cox's ego making him jealous of a baby, Carla and Turk bickering and doing nothing to make up for it, and Elliot for turning him down to be with Paul despite that she's trying to sabotage her relationship with him out of fear.
- Gaussian Girl: We meet TCW in slow motion, with a breeze, and a glow.
- Internal Reveal: Carla learns that Ralphie ate her ring.
- It's All About Me: Part of JD's rant includes telling Cox how narcissistic he is.
- Oh, Crap!: Eliot tries a bit of sexy bondage stuff with Paul, but he trips over a coffee table and gets hurt.
- O.O.C. Is Serious Business: JD, the guy who is usually the dorky Nice Guy they either like or like to pick on, ends up giving everyone a scathing talkdown in response to their criticizing his spending time with Jamie. The fact that Dr. Cox, who would normally respond negatively to JD's talking back with sarcasm or comical anger, is left stunned silent when JD calls out how he's jealous of a baby says something.
- "The Reason You Suck" Speech: JD unloads on everyone.
- Title Drop: It happens rather quickly because we meet Tasty Coma Wife early in the episode.
- Truth in Television: Ralphie clearly has pica, an eating disorder where you eat non-food items.
