The third episode of Quake has the Ranger going through the Netherworld in order to retrieve the Rune of Hell Magic.The episode is composed of six episodes, plus a Secret Level: "Termination Central", "The Vaults of Zin", "The Tomb of Terror", "Satan's Dark Delight", "The Wind Tunnels" and "The Chambers of Torment".

- Cosmetic Award: The 2021 remaster offers the following achievements for this episode:
- Finding the entrance to the Secret Level earns you "The Hot and Winding Road".
- Completing the episode earns you "Beneath the Blue Chambers".
- Dungeon Bypass: "E3M2: The Vaults of Zin" starts with a Silver Runekey visible, at walking distance. Problem is, as you move towards it, the Runekey drops and gets locked in another floor. There is a Loophole Abuse that comes with the fact you can Grenade Jump (in Easy or Normal) or get propelled with the nearby zombies' weak attacks (in Hard or Nightmare) to move fast enough to grab the Runekey before it gets locked, bypassing more than half the level.
- Elevator Action Sequence: "E3M7: The Haunted Halls" has one where you ride a series of floating platforms up and around the level, shooting rockets at switches and killing ogres.
- Fire and Brimstone Hell: Most of the levels have this theme, complete with copious amounts of lava, with an industrial castle theme serving as a backdrop that can make the lava seem like molten steel in a foundry.
- Room Full of Crazy: "E3M7: The Haunted Halls".
- Secret Level: "E3M7: The Haunted Halls".
- To Hell and Back: The episode is designed with an industrial hell theme.
