In flashbacks, Kate marries a friendly police officer named Kevin, who doesn't know she is a fugitive. She calls Edward Mars, who offers to stop chasing her if she's really capable of settling down. However, after a pregnancy scare, Kate realizes she can't stay with Kevin, and leaves him.

On the Island, Locke tells Sayid that Eko was killed by the Monster. During his burial, Locke sees the sun shine on a line of scipture on Eko's stick: "Lift up your eyes and look North. John 3:05." He interprets this as a message.

On Hydra Island, Jack refuses to operate on Ben's tumor. Alex attacks the Others while Kate and Sawyer work, looking for Karl. Juliet tells Kate that Pickett wants to kill Sawyer, and convinces her to persuade Jack to do the surgery. Jack still refuses, but changes his mind when he sees surveillance footage of Sawyer and Kate having sex in the cages. During the surgery, Jack deliberately makes an incision in a vital organ, and tells the Others he will let Ben die unless they allow Kate and Sawyer to escape. Tom interrupts Pickett as he is about to execute Sawyer, and makes him give Kate his walkie-talkie. Jack yells at her to run.


Tropes in this episode include:

  • And This Is for...: Pickett, as he prepares to execute Sawyer:
    Danny: This is for Colleen, you son of a bitch.
  • Call-Back:
    • The story of Kate's marriage, previously mentioned in "Outlaws", is shown.
    • Kate previously mentioned having once taken a pregnancy test in "The Whole Truth".
    • In "Exodus, Part 1," the Marshal remarked that Kate started calling him at his house during the time he was chasing her. We see her calling him at his office, asking him to stop chasing her so that she can settle down.
  • Cliffhanger: The episode ends with Jack holding Ben hostage in surgery and telling Kate to run. The ending was designed to serve as a mini-cliffhanger to end the "pod" of six episodes that started the season, before the series took a 13-week hiatus.
  • Foregone Conclusion: Kate's marriage is obviously doomed, since she has already mentioned that it didn't last very long.
  • Nice Guy: Kevin is an insanely nice guy, making his and Kate's doomed romance all the more tragic.
  • Oblivious Guilt Slinging: The priest at Kevin and Kate's wedding makes a speech about how much he appreciates "Monica's" honesty and how Kevin told him that he loves her because "What you see is what you get", oblivious to the fact that she is a notorious fugitive using an assumed name who has been lying through her teeth to her new husband since the day she met him.
  • Pregnancy Test Plot: Kate takes a test several months after the wedding. It's negative, but the possibility of it being positive freaks Kate out and forces her to accept that she can't continue her life with Kevin.
  • Wham Line: The first indication that there may be a higher power than Ben on the Island.
    Danny: Shephard wasn't even on Jacob's list.
