Emu is still grasping at what his new responsibilities as Ex-Aid exactly entail, when a new player enters the field: Hiiro Kagami, the genius surgeon and wielder of another Gamer Driver. He immediately berates Hojo for what he perceives as a naive approach regarding Bugster operations. On the other hand, Emu cannot abide Kagami's coldblooded treatment of his patients.

This conflict of ethics between the two Doctor Riders will take center stage, even as a new victim of the Bugster Virus appears - Rensuke, who has fallen ill after his lover Asami cancelled their wedding. To make the matters worse, a new foe straight out of a video game comes along for the ride! But the CR will soon find out that they're not the only users of the Gamer Drivers in the play, for a black Ex-Aid stands in their way!

This episode provides the examples of:

  • Excalibur in the Rust: Brave's main weapon, the Gashacon Sword, first materializes as a rusted sword stuck in stone.
  • Giant Squid: Rensuke's Giant Virus infection manifests as one, though actually it's a fairly accurate rendition of a bacteriophage.
  • Hostage Situation: The Alhambra Bugster holds Asami hostage, promising to snuff her out if the Riders will interfere. Ex-Aid is put aback by this, Brave doesn't care.
  • Inappropriate Hunger: What Emu considers Hiiro's habit to eat sweets between operations to be.
  • I Have You Now, My Pretty: The Alhambra Bugster kidnaps Rensuke's fiancee in order to make her his bride.
  • I Want My Beloved to Be Happy: The reasoning behind Asami's decision to break off the engagement with Rensuke - she truly loves him, but believes that she's not good enough for such a great guy like him.
  • Kill Steal: Ex-Aid attempts to finish off Alhambra before Brave has the chance, but while he does get in a good hit, his rival manages to ultimately put the Bugster down.
  • Storming the Castle: Invoked. To guard itself against Brave's assault, the Giant Virus creates a barrier in the shape of a castle wall and starts shooting energy arrows all over the place. Brave still manages to brave through and overtake the Virus.
  • What the Hell, Hero?: Emu is not happy that Hiiro cares little for patients and, for this reason, decides to remain a Kamen Rider.
  • Wrecked Weapon: Genm cuts in half Brave's initial nondescript sword during their encounter.
  • You Can't Thwart Stage One: As revealed in this episode, the Riders can't just immediately transform into Level 2 to bust the viruses, as the patient's body wouldn't be able to handle it. First, they need to remove the Bugster from the patient's body with Level 1, and only after that you can proceed to blasting them in their materialized form with Level 2.

    Gashat Count: Ex-Aid: 1 Brave: 1 Genm: 2
