- The Bad Guy Wins: Zim steals various organs from the kids, including Dib, and convinces the nurse that he is a normal healthy boy. Dib is also mistaken for a "Mooing Cyborg Boy" because of Zim replacing his lungs with a mooing can.
- Big "NO!": Dib lets one out when he realizes that he is the only kid left that has all his organs and Zim is after him next. He lets out another when Zim corners him and lowers the device to steal his lungs.
- Body Horror: Zim stealing and consuming other people's organs, then replacing them with various objects.
- Brain Theft: One of the organs Zim steals is a brain that he swaps with a can.
- The Cameo: Fillerbunny, the title character from one of Jhonen Vasquez's comic books, can be seen in floating in a jar in the skool science lab at the beginning.
- Crazy-Prepared: Dib planned for his x-ray vision goggles to be delivered to the skool at the exact moment he needed them to look at Zim's insides.
- Darker and Edgier: Easily one of the darkest episodes, if not the darkest, of the whole show. It's light on jokes, full of Body Horror, and plays out more like a horror film than a cartoon.
- Downer Ending: Zim's plot to steal the kids' organs goes off with nary a hitch, and to top it off Dib gets mistakenly outed as a freak of nature.
- Karma Houdini: Zim faces no consequences for stealing organs from the kids.
- Lyrical Dissonance: Discussed. Miss Bitters teaches the class about how "Ring Around The Rosie" is allegedly about The Black Plague (It isn't really. This is just a rumor).

- Nice Job Breaking It, Hero: Zim gets the idea to steal everyone's organs because Dib thought it would be a good idea to taunt him and get him thinking that a routine trip to the nurse's office would out the Irken as an alien.
- Nutritional Nightmare: The school cafeteria food is ridiculously awful such as mayonnaise and corn or ketchup and rice.
- Organ Theft: Knowing full well his Irken biology will expose him as an alien, Zim steals the organs of everyone else in the skool.
- Ow, My Body Part!: Zim hurts his "squeedilyspooch" while playing dodgeball.
- Refuge in Audacity: Zim outright admits to Dib's face that stealing everyone else's organs and stuffing them into his own body will help him fool a trained medical professional, even when it's obvious Zim knows nothing about human biology. He's right.
- Say My Name: "TOOOORRRQUE SMACKEEEYYYYY!"
- Shoo Out the Clowns: This is the first episode where GIR does not make an appearance, establishing this episode as one of the darker ones.
