Helga herself is clearly embarrassed, seeing therapy as shameful, but Phoebe is sympathetic saying it's perfectly normal and even helpful.

Her parents dote on her Parody Sue sister to the point that even when she was too young to have the chance to live up to her, she was neglected in favor of her. Arnold was the first person to show her kindness, and she fell for him. Unfortunately, she got teased for it in preschool, forcing her to hide her feelings using violence since in her 4 year old mind, it was the best way to hide it. She didn't believe Arnold would like her after how she acted so she chose to keep that mindset out of fear the only person to be nice to her would reject her. Even in the present day, her parents still dote on Olga and treat her as barely present with her dad focused more on his own success in life and her mom being barely functional on a good day. Dr. Bliss takes sympathy and tells her that while she can't go assaulting people or animals, she has every reason to feel the way she does and she'll be there to help in the future.
