Chris is embarrassed over being fat, so Peter helps get his son in shape. When that doesn't work, Peter takes Chris to Cleveland Brown's brother, who works as a cosmetic surgeon specializing in liposuction. Chris decides not to do it and just stick to getting thinner with diet and exercise, but Peter starts getting plastic surgery; first, to become thin, then to become inhumanly handsome. Meanwhile, Stewie begins gaining weight after eating all the junk food that Chris can't have on his diet.

  • Breathless Nonsequitur: When Peter comes home after making himself handsome, Brian initially doesn't recognize him, thinking he's a stranger barging into their house until Peter steps in front of him.
    Brian: Hey pal, you can't just walk in here without— holy crap, it's Peter.
  • Disproportionate Retribution: In a cutaway, Old Yeller is put down because he accidentally erased an answering machine message.
  • Distracted by the Sexy: Despite being serious with him, Lois can't help but be uncontrollably attracted to Peter after his plastic surgery. To the point of jumping him while yelling at him. She asks Brian if it makes her a bad person to lust after Peter despite being mad at him....and Brian says that yes, it absolutely does.
  • Does This Remind You of Anything?: When a security guard at the grocery store accuses Chris of trying to steal a couple of hams under his shirt (actually because he's fat), he pulls it up to look, and upon realizing Chris was just fat, he childishly makes fun of him and even squeezes his chest, giving the whole thing a sexual harrassment vibe.
  • Dramatic Unmask: After Peter lands himself in the hospital, the doctor warns the rest of the Griffins: "Medical science has only come so far." He unravels the bandages, revealing a Mr. Potato Head face. It turns out it was just a protective mask, and Peter's actual face looks normal.
  • The Fat Episode:
    • Played straight when Stewie's desire to mock Chris by gorging on food in front of him backfires. Stewie gets so fat his arms can't even reach his face and he has to be carried around in a wagon.
    • Inverted with Peter who gets liposuction to become thinner; later he has muscular implants added. An accident near a conveniently placed lard factory brings him back to normal.
  • Fat Idiot: What Stewie basically turns into due to being so obese he doesn't have the energy to make smart comments.
  • "Flowers for Algernon" Syndrome: Peter gets liposuction and a second surgery to appear more muscular and attractive to Lois. The new handsome image goes to Peter's head and he starts neglecting his family to instead hang out at the Handsome Guys club and enjoy the new privileges society packs on him for his appearance. However, Peter while driving is so distracted by his appearance in the rear mirror that it makes him drive off the road and bounced into a lard factory where he swallows a bunch of lard and becomes fat all over again. The family decide it's better if he stays fat.
  • Get A Hold Of Yourself Man: When Brian freaks out about having fleas:
    Peter: Get ahold of yourself! (slaps Lois)
    Lois: Ow! Peter, you're supposed to hit Brian!
  • Irony: Stewie rubbed it in to Chris (who was on a diet) about all the food he was missing out on that he ate too much himself and became obese.
    Brian: Serves you right. You tried to make Chris jealous, now you have an eating disorder.
    Stewie: Just help me up.
    Brian: I would, but my doctor advised me against heavy lifting.
  • Laser-Guided Karma:
    • After spending the whole episode taunting Chris by eating food he wants in front of him, Stewie gets this when he becomes fat and gets an eating disorder.
    • After acting like a jackass throughout the whole episode, Peter crashes his car and ends up falling into a vat of lard.
  • "Not Wearing Pants" Dream: Stewie's dream:
    Stewie: Attention, world leaders. I have 137 nuclear warheads trained on every capital city around the globe. The world is now under my control. But oh, no! I'm naked!
  • Orgasmically Delicious: Stewie fakes having an orgasm to his plate of meatloaf because it's that delicious and to mock Chris for dieting. Brian deadpans "I'll have what he's having."
  • Rhetorical Question Blunder: After muscular Peter calls Lois a "normie":
    Brian: Are you gonna take that?... Lois!
    Lois: What? Oh. Brian. I was I was seeing if the, uh, driveway.
    Brian: That wasn't a sentence. You were ogling Peter like a horny schoolgirl.
    Lois: Oh, I can't help it! I know, I know, he's become a superficial, egomaniacal jerk. But I've never been more attracted to him. Does that make me a bad person?
    Brian: Yes. Yes, it does make you a bad person!
  • Shell-Shocked Veteran: Logan, one of the exterminators.
    Guy 1: Logan, let's go.
    Logan: I can't. I I just can't.
    Guy 2: What's with you, Logan? You look like hell.
    Guy 1: He just watched his wife and kid get carried away by seven million fire ants. I don't wanna meet the man who looks good after that.
  • Snap Back:
    • Stewie's inexplicably back to his former size by the end of the episode.
    • Possibly with Peter not wearing glasses after his second surgery but wearing them at the end of the episode. It's not stated if he had surgically repaired his eyes or if he was simply wearing contacts as part of his new look.
  • Spiteful Gluttony: When Chris attempts to diet, Stewie can't resist gloating by eating sloppily and excessively to his face.
  • There Is No Kill Like Overkill: The exterminators are equipped like a SWAT team and kill the fleas in the Griffin house with machine guns and grenades.
  • Toilet Humour: One of the gifts Peter gets from the beautiful people's club is a pill that makes your bowel movements smell like fresh cinnamon rolls.
  • Took a Level in Dumbass: Stewie turns into a Fat Idiot thanks to his overeating, being so bloated and tired he doesn't have any energy to make smart remarks.
  • True Beauty Is on the Inside: Deconstructed. While Lois has always loved Peter despite him being a poster child for the classic Ugly Guy, Hot Wife trope, it's always been as a form of rebellion against her own upper-class roots and because of Peter's uncompromisingly staying true to himself no matter what. Lois has never really been physically attracted to him, and once he's turned himself into a Brainless Beauty, she can't resist him despite knowing that it's both incredibly shallow and morally wrong.
  • Who Would Be Stupid Enough?: Lois asks, "I mean, what kinda lazy, narcissistic, irresponsible moron would even consider doing something as unbelievably foolish as getting liposuction? Who, I ask you? Who?" Cue a much slimmer Peter walking in.

After his house is infested with fleas, Peter calls upon a team of exterminators that are extremely over-equipped for their job and end up doing more damage to the house than the fleas ever could.
