After leaving Highland Park, Adam and Malik are diverted to rescue David Sarif's niece, who has been captured by a cartel in Juarez, Mexico.

Adam kills several guards on his way into the cartel's stronghold where the woman, Nina Freeman, is being held. When one of her captors threatens to cut off her fingers, Adam smashes through the roof and kills most of the cartel leadership. A standoff occurs between Adam and Nina's torturer, who threatens to kill her if he doesn't release the head of the cartel (whom he's captured). Adam responds by dropping his gun and using the cartel leader as a Human Shield, then stabbing the assailant with his wristblades. He rescues Nina and returns her to her mother in Detroit.

Afterwads, he goes for a checkup at the Detroit LIMB Clinic. Dr. Vera Markovic explains that he's checked out physically and asks if he's still having bad dreams. He makes light of the situation and flashes back to the Sarif attack before affirming that he's fine.

He heads back to Sarif Headquarters and has a meeting with David and Pritchard about the manufacturing plant attack and the secret FEMA base. They discuss whether there was an unseen benefactor behind Nina's kidnapping, but Adam reasons that it was strictly limited to a terrorist group that attacked because the company was seen to be weak. Adam asks to pursue the FEMA lead, but Sarif orders him to focus on Barrett's lead instead.

At the same time, a Humanity Front demonstration in Detroit goes awry when an augmented man detonates a Typhoon in the middle of the crowd, then goes on to attack and kill nearby civilians. While the speaker, William Taggart, and his bodyguards flee, the man kills a reporter and chants the phrase "deus ex" to the cameras.

Adam suffers from nightmares that night and goes back to the LIMB clinic, where he talks with Dr. Markovic about his feelings towards the attack that led to him receiving augmentations. Pritchard radios to tell him about the attack at the rally. Adam heads to the crime scene and investigates, running into a federal agent (and fellow friend in the police force) named Jake Dredger, who antagonizes him before leaving. Adam heads back to the Sarif offices and views video of the attacker, describing his augmentations in detail to the rest of the staff. He pauses to examine a red crosshair the attacker painted with blood on a camera. In a flashback, Adam thinks about his former commanding officer, Quincy Durant, who led a unit of men (including Adam) that sported crosshair tattoos and were trained to shoot first and ask questions later.

Adam is deployed by David to find the location of he hacker who was remotely controlling the augmented man at the manufacturing plant. Malik flies him to a location in Montreal, and they arrive on the roof of a hotel. Adam expresses confusion and asks Pritchard if he has the right intel, but the latter responds that this is where the hacker's source was emanating from. Inside, Adam tracks the source to a hotel room and finds... a woman finishing up her shower. She demands to know who he is, and after he tells her why he's there, she says that she's not a hacker, but the person hackers send after augmented people who go bad.

The woman dresses and introduces herself as Lady Katrina Sutherland, and that she is tracking someone as well. Their conversation is cut short when they come under fire from a sniper across the street. Adam shields Katrina from gunfire, and they decide to make a break for the roof, taking down gunmen and snipers as they get to the top, the terrorist who bombed the rally attacks them. Katrina shoots him down, but he stands back up and reveals himself to be... Durant.

After Adam tries to make small talk, Durant reveals that he is now a force of evolution, before trying to kill the duo with his Typhoon. Adam once again protects Katrina, but is knocked to the ground and nearly killed before Malik appears in her VTOL and begins firing at Durant, who leaps off the building. Katrina tells them that they need to regroup and think of a plan, and Malik jokes that Adam has discovered Lara Croft before they head off. On the way back, Adam has more flashbacks to an incident where Durant was seriously wounded by an augmented war veteran, and was forced to get augmentations himself in order to save his life.

Back at Sarif Headquarters, Katrina reveals that she was led on just like the rest of the group, and was pursuing a group that hired a high-tech hacker, known as Windmill, to lead them in circles. Adam then reveals that the leader of this group is most likely Durant, who he explains went crazy after being augmented, and that Adam has presumed him to be dead after an incident in the past.

Afterwards, Adam allows Katrina to stay at his apartment for the time being, and she relates her backstory of being injured in a horse riding accident and having augmentations installed, then working as a freelancer. They nearly kiss, but are interrupted when Pritchard calls to tell them that there's a situation.

Pritchard reveals that someone has been targeting Sarif Industries employees who were involved with Adam's augmentation procedure. The agent is told that all of the remaining employees are safe, except for Dr. Markovic, who has her security detail ambushed and killed by Durant at the LIMB clinic. She is taken hostage by him before Adam can get to her location. In the aftermath, Pritchard discovers that Jake Dredger had access to the protection detail for the scientists, and knew where Dr. Markovic would be vulnerable.

Adam and Katrina pay Dredger a visit and throw him through a television before interrogating him about Durant's whereabouts. Although Dredger acts defiant, he eventually gives up the information (while being held over the edge of a roof) that Durant is hiding at the Delaney car plant. He tells them that they'll never be able to get in because Durant has an army of mercenaries. He also reveals to Adam that he was the one who rescued Durant after the latter's confrontation with him years earlier, but that Durant has been slowly growing unstable over the intervening years. Dredger also attempts to tell the pair about the Illuminati, but his killswitch is activated and he dies before he can provide any significant information.

Adam goes on a solo mission to rescue Markovic, breaking into the car plant under cover of darkness and knocking out several guards on his way. After he takes down a mercenary who implied he was planning on raping Markovic, Adam is tazed by Durant, who reveals himself to have had further augmentations since their last encounter. Adam has another flashback, this time to the aftermath of the Mexicantown Massacre, and how he was forced to confront and seemingly kill Durant because of the latter's growing insanity.

In the present, Durant tells Adam that "they" (the Illuminati) implanted a mind-control chip in him, but he had Markovic remove it. He claims that he sees the world for what it really is, and that he is going to kill people who stand in the way of the "human revolution". He orders his men to kill Adam, but they are killed in turn by Katrina, who is across the street providing cover fire. Adam narrowly escapes with Markovic before the building goes up in an explosion.

Everyone retires back to Adam's apartment, and David arrives to tell him and Katrina that Markovic is recovering from her traumatic experience. He also reveals that they couldn't find Durant's body in the wreckage. David finishes by saying that Adam will be needed in London the next day before leaving. Afterwards, Katrina surprises Adam in the shower and tells him that they should make the most of the night, and they proceed to consummate their relationship. Meanwhile, at the ruins of the auto plant, two agents discover Durant's body, but he is revealed to be Not Quite Dead and kills both of them before escaping.

The next morning, Adam and Katrina escort David to a meeting in London with Zhao Yun Ru and William Taggart, who attempt to rope him into the Illuminati conspiracy. Despite their attempts to sway him, David remarks in no uncertain terms that he will be declining their invitation and wants nothing more to do with them. Outside the meeting room, Katrina says that she's going to stay behind, and realizes that Adam is still harboring deep feelings for Megan, who he feels he failed and is trying to avenge her. She kisses him goodbye before he goes.

As the VTOL is flying back with David, Adam and Malik onboard, the craft is hit with a missile, causing the cabin to depressurize. David is nearly sucked out before Adam pulls him back in, all the while trading small-arms fire with Durant, who has appeared in a gunship of his own. Malik attempts to level the VTOL out over the River Thames, while Durant hacks into the ship's comm system and goads him. Malik pilots the ship in a loop through the supports of a bridge and flies back up towards Durant. Adam uses the opportunity to jump out of the ship onto Durant's craft, all while the latter expresses surprise at such a stunt.

Adam smashes through the cockpit of the ship and begins beating Durant, then activates his own Typhoon. The resulting explosion seriously injures Durant and causes his ship to crash, while Adam leaps out in mid-air and uses his Icarus Landing System to get down to the ground safely. He radios Pritchard and asks why and how Durant's still alive, and the latter tells him about the dead agents at the auto plant. Pritchard then scans his network and finds Durant's signal, tracking him to the nearby Battersea Power Station.

Malik and David look after their injuries while Adam continues on alone to the plant. He makes his way through the front door, only to be immediately fired upon by Durant, who is now missing an arm and has extensive damage. Durant goads Adam again, then reveals that he captured and imprisoned Katrina before attacking the VTOL. Adam breaks through a nearby wall and finds her chained to the ceiling, strapped with several packs of plastic explosives. She tells him that the bombs have been wired to her heartbeat and are motion-triggered, and that he can't defuse it. Adam turns back around and tells the wounded Durant to disarm it or he will die. In turn, Durant reveals that the bombs are also linked to his mind, so he could kill Katrina with a single thought.

Durant tries to compel Adam to kill himself, and claims he will release Katrina if the latter does so. However, Katrina breaks this stalemate by ripping off the chains and throwing herself at Durant, which causes the bombs to detonate and her to blow up. Jensen is knocked back by the blast, then stands up in shock and surveys the carnage. He finds Durant, still alive but barely breathing, and finally executes him for good with his armblades.

Malik lands the damaged VTOL soon after and tells him that they need to leave. He reports back in turn that there's nothing left of Katrina to recover. Back in Detroit, David tells Adam that there's still more work to be done tracking the mercenaries who attacked the FEMA facility, and Adam wonders to himself whether he's still a man or a machine...
