Basic Trope: A nurse, commonly older and unattractive, who is psychotic and treats their patients cruelly and sadistically.

  • Straight: Nurse Alice treats her patients in a sadistic and cruel manner.
  • Exaggerated:
    • Nurse Alice has brutally killed or murdered every single patient that she has treated or come in contact with.
    • Nurse Alice has murdered an entire hospital of patients.
  • Downplayed: Nurse Alice treats some of her patients cruelly and aggressively, but she's still pretty harmless for the most part.
  • Justified:
    • Nurse Alice is a Sadist.
    • Nurse Alice is suffering from an ambiguous disorder or mental illness (i.e., Borderline Personality Disorder).
    • Nurse Alice is emotionally unstable.
    • Nurse Alice is The Sociopath.
    • Nurse Alice is a psychopath.
    • Nurse Alice is Axe-Crazy.
    • Nurse Alice suffered serious trauma and had a severe Sanity Slippage.
    • Nurse Alice has terrible bed side manner
  • Inverted:
    • Nurse Alice is very kind and caring and treats her patients very well.
    • Hospital Hottie
  • Subverted: Nurse Alice treats her patients cruelly, but it turns out that she's Obfuscating Insanity and pretending to be mean in order to protect her patients from a bigger threat.
  • Double Subverted: Nurse Alice is cruel and mean, but it's later discovered that it was all a facade to trick a bigger threat..but it turns out that her cruel behavior wasn't that much of a facade at all.
  • Parodied: Nurse Alice puts a sign that says "Torture Me" on every single patient that she treats.
  • Zig Zagged: Nurse Alice refuses to treat her patients cruelly, but that's because the patients are nice to her. She'll happily torture and mistreat those who are mean to her. But she doesn't seem to care whether they are nice or mean to her because she loves torturing people.
  • Averted:
    • Nurse Alice may be a mean person but she doesn't treat her patients cruelly or torture them.
    • Nurse Alice is harmless and doesn't mistreat her patients.
  • Enforced: "Nurse Alice is the cruelest nurse and she gets pleasure in torturing her patients."
  • Lampshaded: "I didn't have to torture that patient, but I found pleasure in doing it."
  • Invoked: "But how will my patients know I'm crazy and sadistic if I don't torture my patients and treat them cruelly?
  • Exploited: Nurse Alice treats patients cruelly because it will cause the patients' loved ones to become angry and end up making mistakes that she would be able to take advantage of.
  • Defied:
    • Nurse Alice refuses to torture her patients or treat them cruelly without some good reason to do so.
    • Nurse Alice actually cares about her patients, and wouldn't only she not mistreat them or torture, but she would kill anyone that tries to mistreat and torture them.
    • The administration of the hospital as a less-than-zero-tolerance policy on patient abuse, and Nurse Alice is either kicked out or forced to express her aggression someplace other than the hospital grounds.
  • Discussed: "You've got to watch out for Nurse Alice. She's sadistic, emotionally unstable and she likes to torture her patients and treat them cruelly!"
  • Conversed: "Have you noticed how Nurse Alice has a mean spirited and cruel personality?"
  • Deconstructed:
    • Nurse Alice is a realistically emotionally unstable, sadistic psychopath who genuinely can't pass by a vulnerable patient without being compelled to torture them or mistreat them. The pain and suffering that Nurse Alice causes becomes the focus of the story.
    • Nurse Alice is murdered by her much more dangerous superior.
    • Nurse Alice's patients fight back against her cruel mistreatment and torture.
    • Nurse Alice's patients are disgusted and horrified by her cruel behaviour and actions towards them and they plot with each other to bring her down in the worst way imaginable together.
  • Reconstructed:
    • Nurse Alice's act of cruelty and sadism is highly gratuitous that it forces everyone to reevaluate her as a person, leading to huge differences and changes in her status in the medical field.
    • Nurse Alice's superior is irritated by her cruel and sadistic actions and tendencies, but he decides to keep her around because she's good at the medical aspect of her job and brings results for their Evil Medical Company.
    • When the patients spring their trap Nurse Alice fights back, and the result of destroying her enemies is her becoming even more of The Dreaded, which she of course exploits as one more tool in her sadistic toy box.
  • Played Literally: Mad Doctor: She's easily comparable to a crazy, insane doctor. The only difference is that she's a nurse instead.

You better go back to Battleaxe Nurse. Otherwise, she'll tie you up and torture you!
