- Luigi himself. A very mentally ill man that hallucinates, and is also very skilled in killing his enemies. Despite being a single man, Luigi kills many police, SWAT and innocent people over the course of the game. Of course, Luigi sees them all as cartoon characters. The basement of Peach's Castle reveals he's a serial killer.
- Seeing Luigi's mental state deteriorating over the course of the game. At first, Luigi imagines the mental ward he's in as Bowser's castle and the guards there as Koopa Troopas. During the game he imagines that he's defeating Koopa Troopas to make it back to the Mushroom Kingdom and his brother, when he's actually murdering police and civilians to get back to an apartment in Brooklyn. Things only get worse after he sees that his brother has committed suicide. His mental state reaches an all time low as he imagines himself fighting his way through Koopas and disemboweled zombie versions of himself in a large fiery void filled with floating debris and chunks of buildings. Vinny's voice acting for Luigi during this scene in his stream also adds to this.Luigi!Vinny: I don't even fucking care anymore! Everything I've ever loved... is GONE!!
- The scene where Luigi finds Mario's corpse. He proceeds to break down and hallucinate that he's in a sewer with corpses completely covering the floor. He then come across his own corpse, hung from his feet and missing a large portion of his stomach, guts spilling out all over the floor. He also comes across zombie versions of this as enemies in the next scene.
- Peach's Castle is full of missing posters, bloody tools, and anatomical diagrams. The basement is full of corpses, including Peach's. On the wall where the latter is discovered, not only is there a dead body locked in a cage, but there are drawings near a desk. One is a poorly drawn Peach with the text "I LOVE YOU LUIGI". The other implies Luigi is a serial killer dismembering people in order to gather ingredients for a "cake".
- The ending to the game, where reality falls apart the closer you get to your goal.
- The intro screen to the final level has text scrolling very fast at the bottom of the screen, which is a jumble of messages from Luigi saying things like "What have I done?" and "I did this", clearly feeling massively guilty over the deaths of Mario and Peach. The screen then gradually turns red.
