  • The opening of the game. Four vagrants are arrested and locked in a secret laboratory to serve as guinea pigs for a new drug.
  • Nekro itself! It's like if the devs took krokodil from real life and made it into a Psycho Serum, and it's peddled through the whole of Russia! It's got a Sickly Green Glow, can restore health, and can induce a ferocious berserk state. And the protagonists are frequently injecting themselves with the stuff just to get through. And to refill their charges, they have to extract it from enemies that are downed and convulsing on the ground...
  • The protagonists awake in a dark and ruined laboratory, then flee through sewers full of giant rats, vicious pigs, and zombie-like men. It culminates in a boss battle against a huge, lumbering monstrosity with severe Body Horror, with one arm looking horribly mangled and the rest of him being all grey and decayed, like some sort of Elite Zombie!
  • The Berserk effect of Nekro: the screen is distorted and flashes red, the music warps too, and the animated portrait of the character who is under the Nekro's effect maniacally moves their jaws. There's also the fact that their grab moves during this state ends up becoming a visceral One-Hit Kill of varying flavors. Natasha, for instance, slams her fist into her unlucky victim's chest and destroys their heart in one blow!
  • The player characters suffer from recurring Nekro-induced hallucinations, set in a kind of visceral room, with a giant beating heart in the back, and a skeleton-thing progressively growing. It's the representation of Nekro's addiction and serves as the final boss, using said heart as a throne.
  • The bad ending: the player characters tear their faces while dying of an overdose of Nekro.
  • The BDSM section of the nightclub. The worst part is being locked in a room full of latex covered gimps willingly waiting their turn to be "punished" or even killed by a torturer-executioner.
  • The first boss' introduction, which has the player running through a hallway covered in gore and half-eaten bodies set to low-key Background Music before coming to a suspiciously large arena. While zombie addicts start pouring in from where you came in, the lights start flicking on and off faster and faster until suddenly the music picks up and a gigantic, misshapen mutant rushes in and starts pummeling the crap out of you.
  • Just as the party is about to punish the Premiere for his hand in the events of the game, they start hallucinating the Nekro Dimension again and the Final Boss starts. When they finally win and come back to reality, the Premier is crucified to his desk with knives and the entire room is covered from floor-to-ceiling in blood and gore. Just what the hell were they actually doing during that?
