  • I wonder how the psychiatrist and everybody around him were able to follow the "plotline" inside Malcolm's head, given that all he could know was just lines of the "characters" and some severely restricted bodily movements?
  • Also of some interest is the question of whether Malcolm could have committed a murder if the persona of the "serial killer" or of his friend Rhodes - who is "known" to have killed before—had gained the upper hand.
  • Incidentally, did the presence of the dead cop in the car trunk and of George in the freezer represent yet more alternate-selves of Malcolm's, that were "killed" in preliminary tests of the psychological culling process which the film symbolically illustrates?
    • Though the dead cop and George didn't disappear not long after they were killed like all the other people killed in the film did, with George having been in there for weeks. There's also the Mexican girl who committed suicide, causing Ed to leave the LAPD. It seems likely that there are people in Malcolm's mental world that are not personalities of his, like NPCs in a video game.
  • Why did Ginny suddenly feel cold at the arrival of Rhodes and Maine, even though neither of them is a serial killer personality?
  • Why does the van lose control in the end? Rivers is strangling the doctor and turns to look at the driver, but he's unable to attack the driver while he's still strangling the doctor. Wouldn't the driver simply slam on the brakes and attack Rivers?
