The last thing seventeen year old Dorothy Gale can remember is being a nine year old on her Aunt Em and Uncle Henry's farm in Kansas. Now, eight years older, Dorothy has woken up in an asylum with no memory of the past half of her life.

Patient provides examples of:
- All Just a Dream: It turns out that "Oz" doesn't exist after all. Or not. It does, but Ozma doesn't want to confuse the amnesiac Dorothy so they're pretending she's in an asylum until her memory comes back.
- Cuckoo Nest: Dorothy starts the fanfic in an asylum with no memory of the last several years. According to the doctors, Oz is a fantasy of hers. This is later subverted when it turns out she's not in an asylum. She's still in Oz with her family and friends, but she is suffering from temporary amnesia.
- Foreshadowing: There's quite a bit to the twist that Dorothy isn't in Kansas, such as Eureka being pink and her sigh sounding human.
- Hotter and Sexier: Downplayed. There's a passing reference to sex. Canon is devoid of romance or sexuality.
- Mythology Gag:
- The last sequence pokes fun at the ending to The Wizard of Oz where Dorothy says her friends were in her dream.
- Dorothy and Ozma being in a Bedlam House is ripped from Return to Oz.
- Plot-Relevant Age-Up: Dorothy is aged up to seventeen so that she can have a romantic relationship with Ozma.
- Royal "We": Dorothy meets a girl her age who believes she's royalty and uses the royal "we". Her name is "Ozma" and she is the princess of Oz.
- Slash Fic: Downplayed. It's not the focus but it does have heavy Dorothy/Ozma elements. Dorothy and Ozma are a couple.
- Take That!: At the MGM The Wizard of Oz film for making Oz All Just a Dream and at "All Just a Dream"/"All Just A Hallucination" works in general.
