Because the game wasn't already difficult enough.
- Take a shot whenever you die.
- An extra if because of an aversion of No Ontological Inertia.
- An extra if you get a new Death By _____ badge.
- An extra if it was in the first five levels.
- Yet another if by a Sewer Crab.
- An extra if because you went all Leeroy Jenkins on an obviously impossible and easily-avoidable situation.
- Bonus Round! Add an extra for every five levels you had and every Game-Breaker-level item you had, plus a drink for each healing potion you had identified and unused.
- Just down a new bottle if the number is getting to seem ridiculous.
- Take a sip for every harmful potion that you Quaff-ID, or every helpful potion you (try to) Throw-ID.
- Take a drink every time you encounter a group of three or more enemies.
- Take a celebratory drink whenever you get a badge.
- Take a drink whenever you find a really good item.
- Two if it turns out to be decayed and/or cursed.
- Take a sip for each monster you create by melee-ing an enemy with that ability.
- Two if it's farming flies.
