In the series, there's no real reason given as to why Vader didn't make sure Reva was dead, especially when he's killed people for far less. While he would NEVER admit it, it's possible that Vader actually does feel a bit bad about killing the younglings, and so decided to spare the one survivor. Most likely by intentionally missing her vitals, knowing she would probably survive.

Vader's appearance has not been seen in the trailers, which suggests they might be hiding something about this. Hayden Christensen's RoleReprisal suggests we might see his face, so maybe he's somehow restored his body. Of course, the show will undo this and he will have to go back in the suit, but it would be a neat twist that would confront Obi-Wan with the corruption of his Padawan in a whole new way.

Plus, it would be so unbelievably awesome to see the incredibly slow and clunky duel from the original film get the production values it deserves, perhaps using a digitally aged Ewan McGregor and Hayden Christensen as Vader (essentially bringing ''Film/StarWarsSC38Reimagined'' into the Star Wars canon.)

The little we know of the Grand Inquisitor when he [[WesternAnimation/StarWarsTheCloneWars was still]] [[WesternAnimation/StarWarsRebels the Sentinel]] suggested he was a very status-conscious Jedi (hence his own contempt against erstwhile Padawan Reva), similarly resentful of not getting to advance beyond his desires (as shown in ''WesternAnimation/DarthVaderDarkLordOfTheSith''. Being subjected to the command/oversight of Vader, formerly the "bad boy poster child" of the Order, probably still grates on him (especially when Vader shows himself to be as impulsive as he was when he was still a Jedi, as he did in Part VI). Hell, it might even inform his subsequent fear and choice of BetterToDieThanBeKilled too: beyond his fear of Vader's proficiency in the Sith arts and what he'd do to him, he resented not going on his own terms as prideful Force-user. [[WMG: Obi Wan briefly managed to get through to Anakin in their duel]]
Note that despite claiming that "Anakin is gone. I am all that remains" Vader immediately follows this up (in response to Obi Wan's apology for his state) that "I am not your failure, Obi Wan. You didn't kill Anakin Skywalker. I did." And during all of this, he's bathed in the blue light of Obi Wan's lightsaber. Perhaps in that brief moment, Anakin managed to briefly turn back to the light, but believing he was past redemption he offered redemption to Obi Wan by absolving him of his part in his turn to the darkside.
