High Priestess since she was only fourteen, M'Weru has for years orchestrated the kidnapping and gruesome [[HumanSacrifice sacrifice]] of dozens to the Bloody Tongue, [[DrivenToMadness driving to madness]] all those who would seek to challenge her power. M'Weru sets in motion the Carlyle Expedition and the subsequent massacre to begin with for the benefit of the cult, seducing Carlyle before tossing him away to be devoured by giant rats. M'Weru, in the climax of the Kenya scenario, awakens the Bloody Tongue to conduct an orgy of violence in his name, tearing apart over 200 sacrifices--down to the [[WouldHurtAChild infants]]--in every gruesome way that can be fathomed, summoning disasters that flatten villages and giving rise to Nyarlathotep's own spawn so he may destroy all that exists.
** Ho Fong is the leader of the Order of the Bloated Woman, and easily one of the most reprehensible Nyarlathotep cult leaders in a module abundant with them. Ho Fong attempted to convert his own wife to his side by systematically destroying her life in an attempt to make her dependent on him, and horribly sacrificed her when she remained defiant, [[AbusiveParents driving his young daughter to insanity]] when she witnessed this--whereupon she was tossed into a room to eat bugs for the rest of her life. The worst of his actions coming to light in ''Bloody Botany'', Ho Fong turned his late wife's garden into a place for him to produce lethal poisons and toxins he kills many people with, reducing it to a pit of BodyHorror where countless people are kept [[AndIMustScream alive in indefinite agony]] as the plants grow within them and countless more are mulched and fed to the plants.

** "Shadow Kingdoms": [[WouldHurtAChild 8MM and Sex Machine]] are the leaders of a child {{snuff|Film}} porn ring, kidnapping children to rape and torture them to death while filming the atrocities to sell them. Sex Machine tortures the victims himself, the bodies of over a half-dozen children found under his basement, while 8MM is smugly sadistic about the filming process, proposing they film a video by forcing one of the witnesses he's captured--many more homeless vagrants murdered by the two for being loose ends--to "play" with a kid before murdering them both.

** [[OurVampiresAreDifferent Comte Fenalik]] himself is a [[AristocratsAreEvil wealthy German nobleman]] who doubles as an ancient sadist of a [[VampiresAreRich vampire]]. Fenalik slaughters the lower class of France at his leisure, leaving families butchered and gruesome, terrorizing messages [[CouldntFindAPen scrawled in their blood]], in one case [[DrivenToMadness driving a party full of people insane]] and making them tear each other apart so he can splash in the bloody aftermath. Behind the doors of his manor are no better than when he prowls the night; Fenalik's own servants are subject to every abuse Fenalik can think of, once deliberately tripping up a young servant as pretext for flogging the boy to death. A nightmare even in the shadow of the Reign of Terror, Fenalik's victims clog an entire torture chamber and a legacy of blood dating back to at least 330 AD.

In Kenya, Nyarlathotep impregnates socialite Hypatia to "replace" the child she lost in a botched abortion, intending on entering the world through his own spawn and even killing his own cultists while opting to obliterate humanity through a blood-drenched apocalypse if he can. With little but horrific cruelty driving him, Nyarlathotep is as wicked as ever and proves himself worthy of all his horrible titles over and over.

Living over a century in stasis, Pierce returns to life to murder an investigative party who stumble upon the base he was sealed in, resurrecting his victims as frozen revenants. Upon the investigators coming into the base as well, Pierce torments and attempts to kill all of them, seeing the descendant of his old colleague who sealed him away among them and vowing to make him immortal to spend the rest of his tortured existence isolated, surrounded by the reanimated bodies of his friends.

* ''Harlem Knights'': Arturo Giovanni was a man always second best. Becoming a poisoner SerialKiller and eventually [[SiblingMurder murdering his twin brother]] to steal his wife, Arturo later [[TilMurderDoUsPart murdered her]] in an argument and pledged himself to the Old One Zathog. [[EvilUncle Poisoning his nephew]] Orsino and killing any who objected, Arturo begins mass {{human sacrifice}}s to unleash Zathog while making Orsino's body a gift to the Old One to manifest in. Once this is done, Arturo plans to rule over New York while the rest of the world becomes a hellscape for Zathog to play in and destroy.

** [[DarkMessiah Selim]] is the founder of the Brothers of Skin, a cult worshipping an aspect of Nyarlathotep named the Skinless One, and one of the most twisted of his many awful worshippers. Selim has hundreds horrifically butchered, [[FlayingAlive flayed]] and mutilated alive with rusty knives in offering to the Skinless One, while organizing the hideous murder of whomever transgresses on the cult's affairs. Selim has the organs and skin of his victims stored away so his Brothers may transplant them into themselves to live forever. Selim's most vile creations are the [[BodyOfBodies Skin Beasts]], created through sewing dozens of [[WouldHurtAChild children]] together [[AndIMustScream alive and in agony]] before pouring melted, enchanted flesh on them, binding them together into one amalgamated abomination that can do nothing more than destroy the enemies of the cult.

Upon being discovered, the two are gleeful to torture their victims, even threatening to burn captives alive.

Possessing others to have them rape and murder, allowing them to see what he did before leaving them as broken shells, Putnam gleefully violates and kills the women who catch his eye before attempting to possess the Investigators and start a new murder spree.

Even disposing of fellow firefighters in this way when they find out too much, Hardman's ultimate plot is to offer up all of San Francisco to Cthugha's flames, annihilating the entire city in an eldritch holocaust.

** "Kontamination": Major Wolfram Engel is a vile, sickly-looking Nazi who experiments in exposing people to another realm merely called the Beyond, resulting in a slew of people DrivenToMadness and death from his experiments. Engel devises a way to create a contagious form of madness he infests in a series of captured American soldiers, [[MindRape breaking their minds]], intending to unleash them onto their own Allies and spread it to destroy the minds of the enemy soldiers and have them kill each other in waves. Engel happily demonstrates the virus on an Allied-occupied German town to kill as many American soldiers as he can. [[ChronicBackstabbingDisorder Barely even loyal to his own side]], Engel is happily willing to exploit and even kill his duped allies through his strategies.
** "Three Kings" (part 1 of "Zero Point"): In a setting replete with Nazi {{mad scientist}}s, few are as unrepentantly wicked as [[StupidJetpackHitler Dr. Graf Ernst von Kammerstein]]. The leader of an operation based near a town in occupied Czechoslovakia, Kammerstein murders and reanimates countless dozens in his laboratory for the purpose of creating unkillable SuperSoldiers for Germany. Many are simply added to Kammerstein's ever-growing menagerie of mindless, shambling zombies that Kammerstein keeps locked up in a pen, and other failures literally fill a graveyard. Kammerstein shows a sinister predilection for [[WouldHurtAChild children]] in his experiments, and when a local priest starts saving as many as he can from Kammerstein's wicked clutches, Kammerstein has a man suspected to be him captured and [[CruelAndUnusualDeath tortured to death]]. Utterly without scruples, Kammerstein is willing to use the innocents he hasn't already murdered as human hostages, and may even let the zombies spill out onto the surrounding town to kill everything in their path.

* ''Harlem Unbound'' ("Harlem Knights" adventure): Arturo Giovanni was a man always second best. Having murdered the realtor of the home beneath which lies Eihort's labyrinth, Vanessa picks up men in bars, then taking her victims there and sending them into the Labyrinth to face Eihort's bargain, allowing her greater knowledge through the spawn implanted within her. Vanessa is the one to send the Investigators to meet Eihort, looking for a mass sacrifice to keep her dark pact going, until she tires of her bargain and uses a spell to kill the spawn inside her before skipping out to serve the next dark god.

''[[AC:This Fire Shall Kill]]''
* [[VillainWithGoodPublicity Lieutenant Robert Hardman]], seemingly an upstanding fireman with a series of certificates for saving innocent lives from fires, is in actuality a deranged servant of Cthugha. Hardman [[MindRape destroys the minds]] of his own department with a series of drugs that brainwashes them into becoming servile to Cthugha, and deliberately sets fire to wreak swathes of damage and consume countless human lives while Hardman always arrives just too late to stop it. Even disposing of fellow firefighters in this way when they find out too much, Hardman's ultimate plot is to offer up all of San Francisco to Cthugha's flames, annihilating the entire city in an eldritch holocaust.

''[[AC:The Wail of the Witch]]''
* Mercy Boothe is a WickedWitch from Salem who made a pact with Nyarlathotep. [[HumanSacrifice Sacrificing]] innocent people, especially [[WouldHurtAChild children]], Boothe was caught and executed, but survived thanks using a Gate to bind her soul by the Elder Things. Returning later to take a new body, Boothe has any in her way murdered, even having one obstacle killed by ghouls. Engel devises a way to create a contagious form of madness he infests in a series of captured American soldiers, [[MindRape breaking their minds]], intending to unleash them onto their own Allies and spread it to destroy the minds of the enemy soldiers and have them kill each other in waves. Engel happily demonstrates the virus on an Allied-occupied German town to kill as many American soldiers as he can. [[ChronicBackstabbingDisorder Barely even loyal to his own side]], Engel is happily willing to exploit and even kill his duped allies through his strategies.
* "Three Kings" (part 1 of "Zero Point"): In a setting replete with Nazi {{mad scientist}}s, few are as unrepentantly wicked as [[StupidJetpackHitler Dr. Graf Ernst von Kammerstein]]. The leader of an operation based near a town in occupied Czechoslovakia, Kammerstein murders and reanimates countless dozens in his laboratory for the purpose of creating unkillable SuperSoldiers for Germany. Many are simply added to Kammerstein's ever-growing menagerie of mindless, shambling zombies that Kammerstein keeps locked up in a pen, and other failures literally fill a graveyard. Kammerstein shows a sinister predilection for [[WouldHurtAChild children]] in his experiments, and when a local priest starts saving as many as he can from Kammerstein's wicked clutches, Kammerstein has a man suspected to be him captured and [[CruelAndUnusualDeath tortured to death]]. Utterly without scruples, Kammerstein is willing to use the innocents he hasn't already murdered as human hostages, and may even let the zombies spill out onto the surrounding town to kill everything in their path.

* [[SinisterMinister Reverend Elijah Smythe]], from the "Beneath the Burning Sun" chapter of the ''Shadows over Stillwater'' module, was a former [[HidingBehindReligion religious terrorist]] who cast aside his Mormon faith to instead serve a sinister, dormant [[HumanoidAbomination star-spawn]]. Smythe [[HumanSacrifice sacrifices]] an entire Navajo village to the star-spawn and shoots the one old man who tries to stop him in the gut, while spurring violence in a nearby town to goad lawmen into his clutches to offer their lives to the star-spawn as well. Smythe kidnaps the daughter of the town's mayor and attempts to [[FlayingAlive flay her alive]], attempts to feed all of his own men to the star-spawn when his usual offerings run dry, and even massacres the entire family of a farmer who dared to stand up to him, not even sparing his innocent farmhands and leaving their mangled bodies as a warning to anyone else who one defy him.

Ssulithan [[KillAndReplace murders and replaces]] the real Joshua Meadham, and upon the betrayal of his "daughter" Rose, begins experimenting to create clones, [[PlayingWithSyringes subjecting humans to horrific experimentation]] to perfect a virus, while also organizing terrorist attacks that could kill thousands of innocents. Plotting to obtain access to Mu in actions that could kill millions, Ssulithan intends on unleashing a virus that will [[DepopulationBomb kill 99 percent of humanity]] and turn the remaining few into broken slaves for a new Serpent Empire.
* Rose Meadham is Joshua's traitorous "daughter", actually a [[HalfHumanHybrid hybridized]] serpent-woman named Sashannal who turned on Caduceus to instead worship Tsathoggua. Calling her rival cult the Inner Night, Rose orchestrates countless atrocities while trying to topple Caduceus, [[{{Fingore}} mutilating the fingers]] of two treasure hunters before trying to torture them to death and having hundreds experimented on, skinned or simply sacrificed to [[EldritchAbomination eldritch monsters]]. Rose is casually willing to have half of Calcutta wiped out by a Haftorang Device, ultimately planning to use a similar DoomsdayDevice to ravage the world with disasters and completely exterminate humanity. Failing this, Rose is content to banish Mu and wipe out much of the world with [[GiantWallOfWateryDoom tsunamis]] as her backup.

* Arturo Giovanni was a man always second best. Becoming a poisoner SerialKiller and eventually [[SiblingMurder murdering his twin brother]] to steal his wife, Arturo later [[TilMurderDoUsPart murdered her]] in an argument and pledged himself to the Old One Zathog. [[EvilUncle Poisoning his nephew]] Orsino and killing any who objected, Arturo begins mass {{human sacrifice}}s to unleash Zathog while making Orsino's body a gift to the Old One to manifest in. Once this is done, Arturo plans to rule over New York while the rest of the world becomes a hellscape for Zathog to play in and destroy.

* Lucien Rigault helps the Investigators against the monstrous Comte Fenalik [[PlayingBothSides for his own purposes]]. Seeking Fenalik's knowledge, Rigault helps to bring about the ReignOfTerror to send 10,000 to the guillotine as a form of [[HumanSacrifice sacrifice]] to help [[EldritchAbomination Azathoth]] manifest in Paris so [[GodhoodSeeker he can become a "Supreme Being"]] and achieve ultimate power without caring for the horrific fate that will befall all of Paris.
* [[OurVampiresAreDifferent Comte Fenalik]] himself is a [[AristocratsAreEvil wealthy German nobleman]] who doubles as an ancient sadist of a [[VampiresAreRich vampire]]. Fenalik slaughters the lower class of France at his leisure, leaving families butchered and gruesome, terrorizing messages [[CouldntFindAPen scrawled in their blood]], in one case [[DrivenToMadness driving a party full of people insane]] and making them tear each other apart so he can splash in the bloody aftermath. Behind the doors of his manor are no better than when he prowls the night; Fenalik's own servants are subject to every abuse Fenalik can think of, once deliberately tripping up a young servant as pretext for flogging the boy to death. A nightmare even in the shadow of the Reign of Terror, Fenalik's victims clog an entire torture chamber and a legacy of blood dating back to at least 330 AD.

* "Assault on the Mountains of Madness": SS-Sturmbannführer Matilda Griess is a high-ranking member of the [[{{Ghostapo}} Black Sun occultist group]] who intends on triggering a "psychic scream" apocalyptic event. Upon discovering the Elder City, Griess [[BadBoss has her own men executed]] to keep its secrets and helps to feed souls to [[AndIMustScream trap them in agony]], intending on harnessing the pain to trigger an [[TheEndOfTheWorldAsWeKnowIt apocalypse]] and [[OmnicidalManiac slaughter everyone in the world]] save only those left to serve her and the Black Sun.

* [[AristocratsAreEvil Baron Hauptman]] is the right hand and mentor of the aforementioned Beast, Edward Chandler. A ruthless 700-year-old aristocrat who tormented and murdered the townspeople in his Transylvanian holdings, even allowing his pet Star Vampire to prey upon them, Hauptman is responsible for numerous atrocities of the Brotherhood of the Beast and directs countless sacrifices and widescale horrors to help Chandler rise to power. Surviving by switching bodies and then murdering his own elderly form, Hauptman enjoys nothing so much as causing pain and intends to wipe out the world of man, being described as addicted to the sight of human suffering.

* The aforementioned [[WouldHurtAChild Malcolm Sedny]] is a disturbed young man who, with minimal influence from the lloiger, becomes captivated by the slayings of the insane Bristol and decides to copy them to fulfill his personal fantasies. Sedny brutally [[SerialRapist rapes]] and [[SerialKiller murders]] a series of young boys across London, even being revealed to keep their torn-out pineal glands as [[CreepySouvenir trophies]].

* [[SinisterMinister Reverend Elijah Smythe]], from the "Beneath the Burning Sun" chapter of the ''Shadows over Stillwater'' module, was a former [[HidingBehindReligion religious terrorist]] who cast aside his Mormon faith to instead serve a sinister, dormant [[HumanoidAbomination star-spawn]]. Smythe [[HumanSacrifice sacrifices]] an entire Navajo village to the star-spawn and shoots the one old man who tries to stop him in the gut, while spurring violence in a nearby town to goad lawmen into his clutches to offer their lives to the star-spawn as well. Smythe kidnaps the daughter of the town's mayor and attempts to [[FlayingAlive flay her alive]], attempts to feed all of his own men to the star-spawn when his usual offerings run dry, and even massacres the entire family of a farmer who dared to stand up to him, not even sparing his innocent farmhands and leaving their mangled bodies as a warning to anyone else who one defy him.
