She needed a cybernetic body from a very young age. It is hinted that the government funded her cybernetic body as well as the tedious upgrades as she grew older. The Major fought for the Japanese military in one of the World Wars, which is where the nickname "The Major" was born.

There is no indication that the cybernetic body and her career would be connected; she was comatose and underage, quite incapable of making a binding contract like that. She did serve in the Self Defense Force, but apparently she was nicknamed "Major" even when she was just a grunt. That is, if she actually was. Her skills, leadership abilities, and the remarkable liberties that she took during the flashback episode imply that she was just posing as a common soldier. Her background in the manga indicates that she worked as an irregular for the Department of Defense, so it's possible that she was granted the rank of Major for the sake of convenience, even though she would be unable to use the rank in most of her secret missions, and it became moot once she got recruited to the Section 9, in any case.
