* Comicbook/DoctorStrange's faithful servant is known only as Wong. Lampshaded in a comic ''Defenders'' series when Nightmare asked if 'Wong' were his first or last name -- and followed up Strange's exasperated silence with "You don't know, do you?" (in the Franchise/MarvelCinematicUniverse at least it's implied he has OnlyOneName)
* In ''ComicBook/HuntersHellcats'', it is never established what Heller's first name is, and no one ever refers to her as anything other than Heller.
* ''Comicbook/TheMask'': Lt. Kellaway never earns a first name, which is lampshaded a few times ("Lieutenant, what exactly ''is'' your first name?"). WordOfGod says it's ''Prometheus''. In [[Film/TheMask the movie]] and subsequent cartoon it's "Mitch".
* Half the (large) cast of ''ComicBook/TheWalkingDead'', which necessitated them to be {{named|ByTheAdaptation}} in [[Series/TheWalkingDead2010 the TV adaptation]]. Such main characters include: Glenn, Andrea, Carol, Tyreese, Sophia, Heath, and Negan.
* ''Franchise/XMen'' comics:

* The first name of "Radio/TheLoneRanger" is never revealed, though his last name is known to be Reid.

In older (pre-1900 in North America; pre-1970 in the UK) fiction, a narrator may refer to a character (especially an older or more socially prominent character) by their [[LastNameBasis surname]]. This is because at that time, given names were much less commonly used socially than they are now; a young character may not even know the given name of an older character they are not related to. It was also common in that time to [[SpellMyNameWithABlank blank out]] the names of real people to avoid lawsuits and the like. This is often true in Japanese media set in the modern day as well, as given names are seen as being incredibly personal and sometimes not even used by close friends who still opt to use the surname. Interestingly, the reverse is true if the work is set in some periods of Japanese history where the size of many of the clans that dominated society meant that it was hard to figure out whom one talking about if one just used the surname, so given names were used more instead. In other Asian media, however, one will often hear people referred to by their given names, but almost never by their surnames -- partly because of the low diversity of surnames in many Asian languages.

* In ''Manga/GourmetGirlGraffiti'', Shiina is known only by her surname, despite being one of the three central characters.

** Only one of Shuu's names is revealed (it's unknown whether it's his first name or surname), but considering he had a sister (who is only known as "Shuu's sister"), it's likely he has both a surname and a given name.

* ''Manga/MissKobayashisDragonMaid'': The titular Kobayashi is only addressed by her surname. Neither her maid/ImpliedLoveInterest, adoptive child, best friend, or even her parents refer to her as anything else.

* In ''Ogawa San and I'', the main character's coworker Ogawa is only referred to by her surname, since while the main character has an UnrequitedLove for Ogawa, the two are not especially close. After Ogawa gets married, her husband's last name is never revealed.

** Nozomi's father's surname can be inferred, but he has no name to call his own.

** For the longest time, Dad is simply known as "Koiwai", with even his best friend Jumbo referring to him with his surname. It wasn't until his mother comes to visit in chapter 86 do we get to learn his first name, Yosuke.

* ''Anime/YuGiOhArcV'': Of the many characters who do not have last names (with most of said characters being orphans), Yuto is probably the oddest case. There's never any mention to his family, but he was living a perfectly normal life going to school and all, and all his friends have surnames, so it's weird that we never hear his -- especially since other characters introduce themselves with their full names. It turns out to overlap with {{Foreshadowing}} because [[spoiler:it's a hint that he's one of Zarc's reincarnations and never had biological relatives]], but not having a last name in a modern city trying to live a normal life is... pretty weird.

* None of the sisters in ''Literature/SisterPrincess'' have family names and until the anime Wataru was only ever referred to by a form of 'brother'. This makes knowing whether they're half sisters or full sisters to each other impossible to tell.
* [[BlindSeer Rezo the Red Priest]] of ''Literature/{{Slayers}}'', only known by his title. His great-grandson Zelgadis was this initially; however, his surname (Graywords) got mentioned in an interview with the creator, and it eventually gets mentioned several times.

* In ''Literature/SuperCub'', Koguma is only known by her surname while Reiko is only known by her first name.

* ''Literature/{{Matilda}}'': Nigel, Hortensia, and Lavender's last names aren't revealed despite all the other kids having theirs revealed.

** The Bedoys siblings are the only kid characters with known last names.
** Mr. and Mrs. Bedoys's first names are unknown, and when the books' web animation adaptations still had comments, the creator was asked via comment what Mrs. Bedoys's first name was and [[ShrugOfGod said she didn't know]].

* ''Anime/FinalFantasyVIIAdventChildren'': Denzel is te only heroic character without a surname, barring the Turks (see Video Games below).

** Characters from [[VideoGame/FinalFantasyII the second game]] only have given names. The [[VideoGame/FinalFantasyIII The third entry]] only has them for their main characters, oddly enough.

** ''VideoGame/ShinMegamiTenseiI'': The Heroine [[HelloInsertNameHere can be given any name in the opening]], but no last name. This turns out to be a plot point: she deliberately doesn't tell her last name to the rebel group she leads, and the [[CorruptCop police]] has no description of her, so they end up arresting any woman that shares the same first name with her even if they are completely innocent, trying to find her by process of elimination.

** With the exception of [[VideoGame/StreetFighterII Edmond Honda and M. Bison]] [[note]]whose initial stands for either, Mike or Master, [[DubNameChange depending on the character bearing the name]][[/note]], the majority of the pre-''[[VideoGame/StreetFighterAlpha Alpha]]'' cast initially had only single names. Ken eventually got a canonical last name of Masters in order to set him apart from [[Franchise/{{Barbie}} a certain other Ken]] once the series started getting some serious merchandizing, while characters introduced later ([[VideoGame/StreetFighterAlpha Dan Hibiki, Sakura Kasugano]], [[VideoGame/StreetFighterIII Sean Matsuda]], [[VideoGame/StreetFighterAlpha Karin Kanzuki, "Rainbow" Mika Nanakawa]], [[VideoGame/StreetFighterIV Juri Han]], [[VideoGame/StreetFighterV Laura Matsuda, Luke Sullivan]], etc.) would have official full names established from the get-go, though they're outnumbered by those without. The [[Film/StreetFighter live-action film]] and [[WesternAnimation/StreetFighter American cartoon]] would give full names to the rest of the ''SFII'' characters (such as Ryu Hoshi, William F. Guile, and Chun-Li [[SpellMyNameWithAnS Z(h)ang/Xiang]]), though that only applies to that particular continuity.

* Try naming five characters in the ''Franchise/SuperMarioBros'' universe that have known full names. We’ll wait (although it’s worth noting that Mario and Luigi’s last name is hinted to be, in fact, [[RepetitiveName Mario]]. They ''are'' called the Mario Bros. after all).

* ''VideoGame/AVeryLongRopeToTheTopOfTheSky'': WordOfGod: They exist, but it's never said except for the only known marriage of Gainer's parents.

* ''Franchise/StarWarsLegend'':
* ''Literature/TheIllustratedStarWarsUniverse'': In the official bio, Senior Anthropologist Hoole is known only by his title and last name, with his first name listed on as "indeterminate".
** This continues in ''Literature/GalaxyOfFear,'' where even Hoole's own niece and nephew only know him as "Uncle Hoole." His full name is finally revealed in ''Army of Terror'' as [[spoiler: ''Mammon'' Hoole, one of the two geniuses behind Project Starscream.]]

* In ''Literature/TheStoryOfValentineAndHisBrother'', the twins' mother is simply Myra, or Forest Myra. Even her first name isn't mentioned until Chapter 18, and is only used a handful of times after that.

* ''Film/OnTheBuses'': We don't get to know Betty or Bridget's surnames.

* ''Film/DoctorInTheHouse1954'': We don't get to know Stella's surname.

* ''Film/SleepingDogs'': The main character is referred to with his last name Smith, or "Smithy" for a nickname, while major characters Bullen and Jesperson are never referred to by first names either. [[spoiler:Though in a FreezeFrameBonus when Smith's police file appears, it shows that his first name is Martin]].
