Cartman and his friends broke off their friendship with each other in the "Vaccination Special", so with no one to take advantage of, Cartman might have been forced become a better person to take care of himself. When the future changes and the boys remain friends with Cartman, the latter continues to take advantage of their kindness using the money they donated to him get breast implant. The boys will probably just refuse to give Cartman any more money knowing that he'll just waste their charity on pointless things, which could also lead to Cartman blaming them (along with his mom) for his misery and the boys eventually deciding to cut ties with Cartman, thus explaining Cartman's terrible fate in the reformed future.
