The only indication that it actually happened is that he was court-martialled for the crime, and is no longer a skyjacker. Who the Entitled was, the reasons for it occurring, and why Caine is said to have done it in a fit of rage when we never see him acting out of rage at any point in the movie, is never expanded upon.
