It only costs $200 to get a license to purchase a fully automatic firearm, that's not expensive, especially to a surgeon. It can take 7 to 9 months to get approved, but we don't know how long has passed between when he decides he wants to get a gun and when he gets it. There's also the very real possibility that he bought the gun illegally, there's plenty of people who would be willing to overlook licensing requirements for the kind of money a successful surgeon could throw their way.
