I love 4, it's actually really creepy.
If a tree falls in the forest and nobody remembers it, who else will you have ice cream with?The problem with 4 is that no one without exposure to the game will know that it's actually related to Undertale and not some other work entirely.
Though I kinda prefer something from the game itself, if it cannot be done without spoilers, then we may have to leave it imageless.

she/her | TRS needs your help! | Contributor of Trope ReportI mean, it's certainly a bit of a drastic departure from the art style of the rest of the game, but I don't think too many people are going to assume that the page image for a work's Nightmare Fuel page isn't actually from that work, especially if it were chosen in an IPD thread with several people familiar with the game involved in the decision process.
And, well, it's the part of that specific part of the game that gives away the twist the least, so.

Moon◊I've taken some shots of gameplay, I know they are both fairly heavy spoilers, but out of context, I don't think they'd do much. I don't actually know how to post images yet(some help there would be nice so I can share them) but the two shots are of Chara's Nightmare Face and the first encounter with an amalgamate, just before the fight(When it shifts into a weird globy thing). The captions I'd give them are respectfully "Since when were you in control?" and "Here we are."(named after the True Labs' music).
No caption is honestly better because no context is creepier.

I'd say the Amalgamate one would work better. Chara's Nightmare Face is, well... It's literally the last thing you see in one of the endings, and Chara DOES resemble Frisk so one could assume some weird spoiler-ness from that image, but the Amalgamates, without context, are just... Creepy shit.
10.2 looks goofy, not scary.

I think 15.1 gives the right balance of creepy and avoiding of spoilers (I don't think the idea that Flowey might get seriously damaged is really that much of a spoiler). 15.2 and 15.4 are way too spoilery in my opinion, especially 15.4 (nothing about the final bosses should be posted, even obscured like that).

15.1 or 15.3 work...although, to be honest, the graphics make me wonder how this would be really fitting for such a trope. Must be the lack of context.
I think anything with Flowey is honestly pretty spoilery. The best bet really is an amalgamate, 15.3 being the best, I think. They're scarier than any of Flowey's Nightmare Faces anyway.

See, that's why I like the faces in 4 - without prior context, it doesn't spoil anything more than "there's some freaky-looking shit in Undertale", which... well, you're on a Nightmare Fuel page, so that's not really a shocker. And if you have that prior context, then you aren't really getting spoiled, are you? You already knew what it's from.

Moon◊Image spoilers are much more sensitive than text spoilers. It is common for a troper to arrive at a page, and then decide not to read it. You can't unsee an image, though. No such thing as a page image spoiler warning. Regardless, there should never be spoilers above the example line.
That ATT conversation is about spoiler markup in example entries. Applying it to the image at the top of the page too is a hasty generalization. There are important, fundamental differences.

I still think 4 is context-lacking enough to pass. Just for reference, I ripped all the faces from the game data to a single image file◊, roughly in the order they appeared (they weren't aligned quite this nicely, but nothing a little rearranging couldn't fix).
I rather like the second one on the third row.
