They exhale the tainted smoke over the victim, and when the victim applies the second half of the nerve agent--which has been added to her perfume--it completes the reaction and triggers the nerve agent.

The victim did smoke a cigar before dying, and one theory is that the suspect who opened the box stabbed the cigar with poisoned blade of the penknife he used to open the box.
