At first, Jof's skeleton appears normal, but Jof develops hiccups that rearrange his bones into a spider-like pattern, then a Jolly Roger pattern, and lastly a hammer and sickle symbol. The last one makes the doctor forsake his oath to "above all, do no harm." Jof runs for his life.
