From YKTTW

Daibhid C: Is the trope name a reference to Duck Dodgers (in which this was the name of DD's character in a MMORPG), or is there something earlier that DD was also referring to?

Anon: Honestly, Don Quixote is so much the icon for this trope that we even have a word in the English language that comes purely from his name — quixotic. It's practically offensive that any minor TV bit villain would have this category named after him instead of one of the signature classics (which is itself referenced across show after show).

Xevon: It's a play on the terms Knight-Errant and Error-Prone. That's probably how it came about Duck Dodgers. As for quixotic, that's already covered by the Other Wiki.

Gloating Swine: Removed Javier. Even the example notes that he's completely not this at all, and his unique personal quirks make him no more of a Bunny-Ears Lawyer than anyone else in Advance Wars.

Removed A Song of Ice and Fire because I can't think of a single good example of a character who is ineffectual, self-deluded, and commonly recognized as such.
