Micah: The film of Charlies Angels was mentioned as a possible borderline example. It's been a long enough time since I actually saw it that I don't remember if it qualifies...
Paul A: It definitely hits points 1 and 2. Point 3 is the one I'm not sure about: it's been a while since I saw the movie, too, but my recollection is that there's no second Caper; by the time the Angels figure out what's really going on, there's no time to plan anything elaborate, so they just go after the villain directly.
